Chauffeur booking form template

A chauffeur booking form should collect the contact, pickup date and place, destination, passenger count, luggage and relevant flight or child-seat details. Use the free printable PDF or CSV field list to prepare a request for review. Submitting the form is an enquiry, not a confirmed booking or an instant price guarantee.

No signup required. Blank working files; adapt them to your operation and applicable rules.

The fields a chauffeur booking form needs

Collect enough information to review the request. Distinguish booker from passenger and pickup from flight arrival. Required fields here are suggested operational choices.

Field blockWhat to record
Contact and communicationCollect the booker name, email and operational contact number. Request a separate passenger contact only when needed. Explain how details will be used; keep marketing optional.
Place and timeCollect pickup date, local time, time zone where relevant, full pickup location and destination. Include known meeting points, additional stops and return details separately.
Passengers and luggageAsk for the number travelling and luggage quantity or size so the operator can assess vehicle suitability. Capture child-seat requirements as a service request that must be checked, not an automatic promise that a particular seat is available.
Flight and changesFor an airport pickup, request the airline flight number and arrival date. Make clear whether the entered time is flight arrival or requested pickup. Ask the customer to report itinerary changes through the stated contact route.
Privacy and reviewPlace a relevant privacy notice near the form. State that availability, vehicle suitability and price will be reviewed before confirmation. Do not collect payment-card numbers, identity documents or detailed medical information in a general notes field.

Putting the form on your website

  1. Choose the fields you need

Use the CSV columns as a specification for your form tool. Review required fields, provide visible labels and show errors beside the affected field.

  2. Explain the next step

Label submission as a request for quote or availability review. Acknowledge receipt and explain how to correct the itinerary. Submission alone is not a confirmed booking.

  3. Test the delivery path

Submit a fictional request through your chosen form tool, check that the office receives all fields and verify a failed submission is visible. Connect the workflow only through a supported process you have configured and tested; the CSV is not an integration or embeddable widget.

Consent and privacy wording by market

United Kingdom

ICO guidance distinguishes consent from other lawful bases, including necessary steps requested before entering a contract. Choose the basis that reflects the actual processing. A compulsory marketing checkbox is not a substitute for explaining how a service request is handled.

Ireland

The Data Protection Commission calls for clear information about the organisation, purposes, lawful basis, sharing, retention and individual rights. Adapt the privacy notice to your real operation and suppliers; generic wording cannot describe a process you have not established.

United States

FTC business guidance recommends collecting only information needed for the service, controlling access and disposing of it when no longer needed. Check applicable state requirements separately. This field list does not certify compliance with every US privacy law.

Canada

The privacy commissioner meaningful-consent guidance emphasises understandable purposes, sharing and choices for unnecessary processing. Explain the actual request workflow and review which federal or provincial law applies. Do not bundle optional uses into the essential booking request without considering those requirements.

Where web form bookings land in 4fleet

Assign someone to review details, assess vehicle suitability, quote and communicate the decision. Keep one request reference across calls and emails.

4fleet enquiry records support a website source, alongside email and phone, and the office can review and quote the request in the enquiry workflow. Enter a website request through the supported intake process you have configured and tested. The field list does not create that connection or provide a custom form builder, embeddable booking widget or instant online booking. The figures show enquiry review using fictional Ashford data.

      Booking form questions

What should a chauffeur booking form ask?

Ask for contact details, date and time, route, passenger and luggage needs, plus relevant airport or child-seat information. Collect only information needed to assess and arrange the requested service.

Should the form show a price?

Only if you can explain and support the price shown. Otherwise request the details and quote after review. A submitted enquiry should not imply availability or a guaranteed fare.

How do I collect flight numbers?

Use a labelled airline flight-number field and a separate arrival date for airport work. Explain that flight arrival and passenger pickup are different times. Leave the field optional for journeys that are not airport pickups.

Do I need a consent checkbox?

Not automatically for every purpose. In UK and Irish GDPR contexts, choose the appropriate lawful basis for arranging the service and explain it. Optional marketing is a separate purpose; review its consent requirements independently.

What happens after a client submits the form?

The office should acknowledge the request, check missing details, review availability and provide the quote or next step. The PDF and CSV do not submit data or create a 4fleet booking by themselves.
